        Welcome to the health information page for CPISD.   I am your District Nurse, Dayna Gwaltney.  I hope everyone had a great summer and you are ready to return to school. I am looking forward to meeting all the students, staff, and their families. Please feel free to reach out to me if I can assist you in any way.

I will be setting up screenings for vision, hearing, and spinal for the age specific groups rather quickly. If there are any concerns with a student’s screening, a referral will be sent home and asked that it be returned to the nurse’s clinic after following up with the doctor. 

Please make sure your student is up to date on all state mandated vaccines and please provide a copy of the updated vaccination record to the nurse’s clinic. You may reach out to your child’s physician or myself if you have any questions regarding immunizations. 

If your child has any health conditions or requires medication to be taken at school, please come visit with me at the nurse’s clinic. I am here to help keep your child healthy and ready to learn. I am looking forward to meeting everyone.
